Referrals for an examination must be initiated by a physician or an appropriate agent of the physician (e.g., nurse practitioner, physician's assistant). As a result, your physician's office will typically contact us directly to schedule your appointment.

On occasion, your physician may instead ask you to call us and make an appointment, in which case you should call our scheduling department between 7:30am and 5:30pm at (425) 486-8868. (Toll free in Washington State: 1-800-282-6516). Our schedulers will help you select from one of our 14 laboratory locations and offer you further instructions to prepare for your exam.

Please have the following information available:

• Patient's full name and date of birth
• Type of exam
• Indications (symptoms)
• Type of insurance
• First and last name of the referring physician

If the exam requires fasting, we will do our best to schedule a morning exam.
